WHO WE ARE

Supported Living is an opportunity

for adults with developmental disabilities to choose where, how and with whom they live. Persons in supported living receive personalized supports necessary to maintain their own private home.

WHAT WE DO

A Supported Living Coach provides training

and assistance in various areas, helping an individual acquire the skills necessary to live and function as independently as possible in their home and community.

WHO WE SERVE

You may be eligible for Supported Living services if

You live in an area where the services are provided, are a Client of the Agency for Persons with Disabilities (APD) who are enrolled in the Consumer-Directed Care Plus (CDC+) program, iBudget Florida waiver, or in Pre-enrollment Status must meet the following eligibility criteria to receive Supported Living services:

  • Age: 18 years of age or older
  • Medicaid: A Medicaid recipient or eligible to receive Medicaid services
  • Living Arrangements: Currently living in their own home or apartment, or have plans to move into their own home within the next 90 days
  • Need for Support: Must require training, assistance, or other supports to live as independently as possible in their own home and community
